چکیده
Background: An increased femoral anteversion angle (FAA) is a predisposing factor for recurrent patellar dislocations (RPDs), and combined procedures including derotational distal osteotomy (DDFO) have been shown to be good options. Purpose: To investigate the safety effectiveness of DDFO on clinical radiological outcomes treat RPDs with an FAA. Study Design: Systematic review; Level evidence, 4. Methods: This review was performed according PRISMA (Preferred Reporting Items Reviews Meta-analyses) guidelines. We searched 7 databases articles from inception March 10, 2023, that reported after in patients RPD Two reviewers independently extracted data assessed study quality. Outcomes evaluated were functional scores, redislocation rates, complications, satisfaction, parameters. A meta-analysis pool as mean differences (MDs) 95% confidence intervals (CIs). Results: Included 8 studies 189 knees 183 patients, patient age 22.4 years follow-up 33.4 months. The preoperative FAA ranged 31° 42.70°, postoperative 10° 19.08°. Significant improvements found Kujala score (MD, 26.96 [95% CI, 23.54 30.37]), Lysholm 26.17 22.13 30.22]), visual analog scale pain –2.61 –3.12 –2.10]), Tegner activity 1.33 0.86 1.79]). No subluxation or occurred. overall complication rate 10.6%, most complications (60%) limited knee (20%). satisfaction 83.3%. tilt significantly decreased 40.7° ± 11.9° 20.5° 8.7° 26.35° 6.86° 11.65° 2.85° 2 studies. Conclusion: Combined safe effective treatment by addressing both torsional malalignment. However, because lack comparisons, it remains investigated when should such patients.

Patellar instability treated with distal femoral osteotomy.
BACKGROUND Patellar instability can cause significant disability in both pediatric and adult patients, and it is associated with several factors including genu valgum. In this study, we describe the role of a lateral opening wedge distal femoral osteotomy (DFO) combined with lateral retinacular release in addressing genu valgum with associated patellar instability. The rationale for this approa...
متن کاملLong-term changes in femoral anteversion and hip rotation following femoral derotational osteotomy in children with cerebral palsy.
BACKGROUND Excessive femoral anteversion is common in cerebral palsy (CP), is often associated with internal hip rotation during gait, and is frequently treated with a femoral derotational osteotomy (FDO). Concerns exist regarding long-term maintenance of surgical outcomes. Past studies report varying rates of recurrence, but none have employed a control group. METHODS We conducted a retrospe...
متن کاملSingle Cut Distal Femoral Varus Osteotomy (SCFO): A Preliminary Study
Background: Genu valgum usually originates from a deformity of distal femur that is often corrected by distal femoralvarus osteotomy. The osteotomy includes both components of angulationcorrection and translation because the siteof osteotomy is not commonly at the apex of deformity. Improvement of patellar tracking not only depends on valguscorrection, but also it may be partially due to centra...
متن کاملFemoral Derotation Osteotomy Technique for Excessive Femoral Anteversion
Excessive femoral anteversion may lead to increasing stress placed on the anterior acetabulum and soft tissues, which can predispose to intra-articular hip pathology. By addressing the excessive femoral anteversion in combination with intra-articular hip pathology, the results will be durable over time. This technique details how to perform a femoral derotation osteotomy for excessive femoral a...

Osteotomy procedures around the knee can alter the biomechanical axis of the knee, thereby shifting the load from one compartment to another. This can be used to unload an arthritic painful compartment or to decrease the weight experienced by a cartilage restoration procedure. Recent clinical and biomechanical studies have emphasized the importance of correcting the weight bearing axis in a car...
